Well, most of the time one of three things happen when someone powers up.

A: The person WANTS to see the more powerful form because they think it's a bluff or that they might then present a challenge.

B: Fear inducing (somehow).

C: Emit a powerful barrier that somehow pushes people and attacks away that is far more powerful than the person can actually make when transformed.

I ask that same question.
The heroes don't always get hit.
I remember one episode of DBZ where Goku was going SS3 on Majin Buu, and I counted over the course of two episode(about 56 minutes)Goku spent 26 of those minutes(more than half)sitting in a constipated pose screaming his head off.

It's such a weird rule. Unless of course, the villain could power up in a single second. That would skip the whole problem entirely!

Sometimes they do hit the enemy[or hero] when when charging up, it always makes them look cheap.
